What to Expect: In home care
Referral Received
Once your healthcare provider or specialty pharmacy submits a referral, our team quickly begins matching you with qualified nursing talent in your area. We ensure the nurse assigned is equipped with the right skills and experience for your specific care needs.
Scheduling & Coordination
We’ll reach out to coordinate your first visit. During this time, we’ll also review your therapy plan and answer any initial questions you may have about the process, your treatment, or the nurse who will be assigned to you.
Nurse Assignment
You’ll be matched with a nurse who is specifically skilled in the therapy or treatment you require. Whether it's infusion therapy, chronic condition management, or a specialized procedure, we ensure that the nurse has the expertise necessary to provide the highest quality care.
Your First Visit
On your first visit, your nurse will thoroughly review your therapy or treatment plan, explain the process, and address any concerns. We prioritize making sure you feel comfortable, informed, and supported as we start your care journey.
Ongoing Support
We maintain open communication with you and your healthcare team to ensure that your therapy or treatment is being delivered safely and consistently. In coordination with your provider, we schedule follow-up visits as needed to monitor your progress and make any necessary adjustments. Should any questions or concerns arise, we are here to provide ongoing support to prioritize your well-being.
